Utah Safety Administrative Management System (SAM)
Environmental Health & Safety has used various iterations of the EHS Assist software platform provided by OnSite Systems since approximately 2010. In 2018 a new web-based version of the system was launched by the vendor based on extensive feedback from University of Utah users of the system. The new client was implemented and branded as the University of Utah Safety Administrative Management System (aka SAM). The SAM system is now utilized for tracking of chemical inventories, unwanted hazardous materials management, safety related documents, safety inspections, safety equipment surveys and testing (fume hoods, emergency eyewash, autoclaves, etc.), industrial hygiene, job hazard analysis, incident management, laboratory closeouts, radiation safety, biosafety, field safety, etc.. The system is part of a continuous improvement process the continues to expand the uses of the system as well as evolve, improve, and streamline already present functions.
The SAM system is the core of the OHSMS, however occupational medicine services such as medical surveillance and clearance are tracked using a software package provided by OpenRange software. Moderated links are present between SAM and OpenRange which direct users to needed Occupational Medicine services such as hearing testing, respirator use medical clearance, vaccinations, etc.

Within 24 hours of an incident, complete the Employer's First Report of Injury or Illness Form and submit it to the Absence Management Team within 24 hours (Official Form 122, State of Utah, Labor Commission, Division of Industrial Accidents). Incidents are reported annually on OSHA Form 300A.

If the injury or illness is disabling or serious, an injured/ill employee or supervisor must contact Environmental Health and Safety (EHS) immediately.
There are three incident reports used by the university:
• Report injuries and illnesses on the job with the First Report of Injury or E-1 form.
• Report property damage and injuries or illnesses involving students or campus visitors using Campus Incident/Accident Report form.
• Report near misses, chemical and biological spills and releases using EHS incident report.
